Worldwide, around 2.25 billion cups of coffee are consumed daily. That works out to more than 820 billion cups of coffee every year, making it one of the most popular beverages globally. In many countries, coffee is more than just a drink.
It’s part of daily routines, social gatherings, and workplace culture. Many people enjoy coffee because it contains caffeine, a natural stimulant that helps increase alertness and reduce feelings of tiredness.
In recent years, researchers have become increasingly interested in whether coffee may also influence long-term health. Several studies have suggested that moderate coffee consumption may be associated with a lower risk of conditions such as type 2 diabetes, cardiovascular disease, and certain liver diseases.
This latest study investigated whether regular coffee consumption was associated with a lower risk of serious liver disease and better long-term liver health in more than 354,000 adults.
Why Is Liver Health So Important?
The liver is one of the body’s hardest-working organs. It performs hundreds of essential functions, including processing nutrients from food, regulating metabolism, producing bile to help digest fats, storing vitamins and minerals, and removing waste products and toxins from the blood.
When the liver becomes damaged, these important functions can gradually become less effective. In many cases, liver disease develops silently over many years before symptoms become noticeable.
One of the most common liver conditions today is metabolic dysfunction-associated steatotic liver disease (MASLD), formerly known as non-alcoholic fatty liver disease (NAFLD).
MASLD occurs when excess fat builds up in the liver and is closely linked to obesity, insulin resistance, type 2 diabetes, and metabolic syndrome. Left untreated, it can progress to inflammation, fibrosis (scarring), cirrhosis, liver failure, and, in some cases, liver cancer.
Because the risk of insulin resistance and metabolic dysfunction often increases during midlife and after menopause, maintaining good liver health becomes increasingly important. However, liver disease can affect both men and women, making it a significant public health concern worldwide.
What Did the Researchers Investigate?
The researchers wanted to find out whether people who regularly drank coffee had a lower risk of developing serious liver disease. They also investigated whether liver scans and blood protein analyses could help explain any associations they observed.
The study included 354,764 adults from the UK Biobank, one of the world’s largest long-term health research projects. Participants were followed for a median of approximately 13 years, during which researchers recorded new cases of cirrhosis, liver cancer, and liver-related death.
Participants reported their usual coffee consumption, including whether they typically drank caffeinated or decaffeinated coffee and whether they usually added sugar or artificial sweeteners.
To gain further insight, the researchers analyzed MRI scans from a subgroup of participants to measure liver fat, inflammation, and fibrosis (scarring). They also examined blood protein profiles (proteomics) to explore possible biological mechanisms linking coffee consumption with liver health.
What Did the Study Find?

The study produced consistent findings across several different measures of liver health:
A Lower Risk of Serious Liver Disease
The researchers found that people who drank more coffee had a lower risk of developing cirrhosis, liver cancer, and dying from liver-related causes during the follow-up period than those who drank little or no coffee.
These associations remained even after the researchers adjusted for a wide range of factors that could influence liver health, including age, sex, body mass index (BMI), smoking, alcohol consumption, physical activity, dietary habits, education, and existing health conditions. This strengthens confidence that the findings were not simply explained by differences in lifestyle or overall health between coffee drinkers and non-coffee drinkers.
Healthier Liver Scans
To explore whether coffee consumption was associated with measurable differences in liver health, the researchers analysed MRI scans from a subgroup of participants.
Compared with people who drank little or no coffee, regular coffee drinkers generally had:
- Less fat stored in the liver
- Lower levels of liver inflammation
- Less fibrosis (scarring)
These findings suggest that coffee consumption was associated with healthier liver tissue as well as a lower risk of developing serious liver disease.
Because excess liver fat, inflammation, and fibrosis are key features of progressive liver disease, the MRI results provide additional biological evidence supporting the overall findings of the study.
Evidence from Blood Proteins
They also analysed blood protein profiles (proteomics) to investigate whether changes in specific biological pathways could help explain the associations they observed.
They identified differences in several proteins involved in liver function, inflammation, metabolism, and tissue repair. Overall, these protein patterns were more favorable in people who regularly consumed coffee.
Although these findings cannot prove that coffee directly caused the observed changes, they strengthen the overall evidence by showing that the associations were supported by measurable biological differences rather than disease outcomes alone.
Caffeinated and Decaffeinated Coffee
Interestingly, the researchers found that the associations with better liver health were seen in people who drank both caffeinated and decaffeinated coffee. This suggests that the potential benefits may not be explained by caffeine alone.
Coffee contains hundreds of naturally occurring compounds, including polyphenols, chlorogenic acids, and other bioactive substances, many of which are thought to have antioxidant and anti-inflammatory properties.
The researchers believe these compounds may work together to help support liver health, although further studies are needed to better understand their individual roles.
What Could This Mean for You?
This study adds to growing evidence that regular coffee consumption may be associated with better long-term liver health. While it cannot prove that coffee directly protects the liver, the findings suggest that coffee could form part of a healthy lifestyle that supports normal liver function.
For most healthy adults, moderate coffee consumption is generally considered safe and may offer health benefits beyond simply increasing alertness.
However, coffee is not a substitute for other lifestyle habits that are known to support liver health, such as maintaining a healthy weight, eating a balanced diet, being physically active, limiting alcohol consumption, and managing conditions such as obesity, insulin resistance, and type 2 diabetes.
Study Limitations
Although this was a large, well-conducted study with a long follow-up period and additional MRI and proteomic analyses, it has several limitations that should be considered.
- This was an observational study. It can identify associations but cannot prove that coffee directly reduced the risk of liver disease or caused the biological changes observed.
- Coffee consumption was self-reported. Participants reported their usual coffee intake, which may not always accurately reflect long-term drinking habits.
- There was no distinction between different brewing methods*. Researchers did not record whether participants drank filtered coffee, espresso, French press, instant coffee, or coffee made using capsules. Because brewing methods can alter the levels of certain compounds in coffee, it remains unclear whether the preparation method influenced the findings.
- The study did not account for where the coffee beans were grown or the type of beans used. The composition of coffee can vary depending on region, climate, altitude, bean variety, processing, and roasting. This means the biological effects observed in the study may not be identical across all coffees.
- The study population was primarily from the United Kingdom. Although the large sample size strengthens the findings, the results may not apply equally to populations with different lifestyles, dietary habits, or patterns of coffee consumption.
- Other lifestyle factors may still have influenced the results. Although the researchers adjusted for many potential confounding factors, it is impossible to account for every difference between coffee drinkers and non-coffee drinkers.
* – Interestingly, a separate 2026 UK Biobank study that specifically examined coffee preparation methods reported that filtered coffee was associated with more favorable markers of biological aging than instant coffee. These findings suggest that brewing method may influence some health outcomes, although further research is needed to confirm these associations.
