For a long time, creatine was primarily known among athletes and people who exercised seriously to improve their performance. For non-athletes, there seemed little reason to give creatine much thought.
But creatine isn’t only relevant to athletes — and its role in the body goes well beyond sports performance.
Your body produces creatine naturally to help supply fast energy right where it’s needed. We usually think of muscles needing that fuel, but your brain is an energy hog too—it relies on this rapid energy system to handle everything from focus and memory to just making it through your day.
This becomes particularly interesting as we get older. From midlife onwards, maintaining muscle becomes increasingly important for strength, mobility, and independence, while changes in memory and thinking can become another concern for both men and women.
So, could something best known as a sports supplement have a role in supporting both?
It’s an intriguing possibility — and one researchers decided to look at more closely.
Why Are Researchers Looking at Creatine and the Brain?
The fact that most of your body’s creatine is stored in your muscles helps explain why it has traditionally received so much attention in sports and exercise research. But creatine is also present in the brain, where it helps meet the considerable energy demands of brain cells.
This raised an interesting question: if creatine can help to support energy availability in muscle, could it also influence how the brain functions as we get older?
How Was the Research Conducted?
Rather than carrying out a new clinical trial, the researchers conducted a systematic review of existing research on creatine and cognitive function in older adults.
They searched several scientific databases for studies involving adults aged 55 and older that examined the relationship between creatine and cognition. The researchers considered studies looking at both creatine supplementation and creatine obtained through the diet.
To be included, the research also had to assess at least one aspect of cognitive function, such as memory, attention, processing speed, or executive function — the mental skills we use to plan, concentrate, make decisions, and manage everyday tasks.
After screening the available research, only six studies met the criteria for inclusion.
The studies weren’t all designed in the same way. They varied in how creatine intake was measured or supplemented, how long participants were followed, and which aspects of cognitive function were tested.
That variation is important, because it means the researchers weren’t comparing six identical experiments. Instead, they were piecing together evidence from different approaches to see whether an overall pattern was beginning to emerge.
What Did the Researchers Find?
Overall, the findings were promising, but not conclusive.
Five of the six studies found a positive relationship between creatine and at least one aspect of cognitive function. The benefits were not identical across every study or every cognitive test, but improvements were reported in areas including memory, attention, and processing speed.
Some of the findings also suggested that the effects of creatine may depend on the individual. People with lower usual creatine intake, including those who consume little or no meat, may potentially respond differently to supplementation than people whose diets already provide more creatine.
However, the small number of studies and the differences between them make it difficult to determine exactly who might benefit, which aspects of cognition might improve, or how much creatine would be needed.
So, while the results provide an encouraging signal that creatine could have a role in supporting cognitive health in later life, they do not yet prove that taking a creatine supplement will improve memory or prevent age-related cognitive decline.
What Could This Mean for Brain Health as We Age?
The findings are interesting because the brain’s energy needs don’t decrease simply because we get older. In fact, aging can bring changes in the way brain cells produce and use energy, which may make maintaining an efficient energy supply increasingly important.
Creatine acts as part of the brain’s rapid energy system, helping regenerate ATP — the molecule cells use as their immediate source of energy. Think of it a little like a home with solar panels and a battery. ATP is the power being used, while the creatine system helps keep that power available by rapidly topping up the supply when demand increases.

That could help explain why researchers are interested in creatine for cognitive health. Tasks involving memory, concentration, and problem-solving all require energy, and having more creatine available may help the brain meet those demands more efficiently.
But there is an important distinction here. The review suggests that creatine may support certain aspects of cognitive performance in older adults. It did not show that creatine prevents dementia, stops cognitive aging, or protects the brain from disease.
Those are much bigger claims — and the research simply isn’t there yet.
What the evidence does suggest is that creatine may have a role beyond the muscles, and that its potential effects on the aging brain deserve further investigation.
What About Creatine and Muscle After 40?
The potential brain benefits may be receiving more attention, but creatine’s relationship with muscle is much better established.
As we get older, we gradually lose muscle mass and strength. This process can accelerate later in life and, if it becomes severe, contribute to sarcopenia — the age-related loss of muscle mass, strength, and physical function.
This matters for much more than how muscular you look. Maintaining muscle helps support balance, mobility, bone health, metabolic health, and the ability to remain physically independent as we age. And it works both ways: strong, healthy bones provide the stable framework your muscles need to support movement, strength, and mobility.
Research suggests that creatine supplementation can help older adults gain or preserve lean muscle mass and improve strength, particularly when it is combined with resistance training.
If you’re new to resistance exercise, prefer a gentler starting point, or are returning to exercise after an injury, Aqua Pilates can be one way to begin rebuilding strength with less impact on the joints. That last part is important. Creatine isn’t a replacement for training; it supports the work your muscles are already doing. Resistance exercise provides the stimulus for your muscles to adapt, while creatine may help them produce energy more efficiently and get more from that training.
This makes the emerging research on the brain particularly interesting. Creatine could potentially have relevance to two things that become increasingly important with age: maintaining our physical strength and supporting how our brain functions.
The evidence for those two roles, however, isn’t equally strong. We know considerably more about creatine and muscle than we currently do about creatine and the aging brain.
What Does This Mean for You?

If you already use creatine as part of your exercise routine, this research gives us another interesting reason to study what it may be doing beyond your muscles.
If you’ve never considered creatine because you thought it was only for athletes, this research may give you a reason to look at it differently. But it doesn’t mean everyone over 40 should start taking a creatine supplement. The review doesn’t tell us the ideal dose, who is most likely to benefit, or whether taking creatine over many years protects cognitive health.
For now, the most practical reason to consider creatine after 40 remains its established role in supporting muscle and strength, particularly alongside resistance exercise. Any potential benefit for your brain should be viewed as promising research rather than a reason to start supplementation on its own.
If you’re considering taking creatine, particularly if you have a medical condition or take regular medication, it’s worth discussing it with your healthcare professional first.
Limitations of the Research
The biggest limitation is simply how little research is available. Only six studies met the criteria for the review, so the conclusions are based on a relatively small body of evidence.
The studies also differed considerably in how they investigated creatine. Some examined supplementation, while others looked at creatine obtained through the diet. They used different amounts of creatine, different study durations, and different tests to measure cognitive performance. This makes it difficult to compare the results directly or determine whether one particular approach is more effective than another.
The participants were also adults aged 55 and older. While the findings are relevant to healthy aging, they cannot tell us whether the same cognitive effects would be seen in people in their 40s.
Larger, well-designed clinical trials are needed to establish whether creatine genuinely improves cognitive function in older adults, which aspects of cognition might benefit, and who is most likely to respond.

Creatine may have spent decades being seen primarily as a sports supplement, but the research suggests its potential role could extend beyond muscle and physical performance.
For maintaining muscle and strength as we age, particularly alongside resistance training, the evidence for creatine is already relatively well established. Its potential effects on the aging brain are much less certain, but this review provides an interesting reason to keep investigating.
While five of the six studies found a positive relationship between creatine and at least one aspect of cognitive function, it is too early to say that taking creatine will improve memory or protect against cognitive decline, especially considering such a small and varied evidence base.
For now, creatine looks increasingly relevant to healthy aging — not because we know it can keep both our muscles and brains younger, but because there is enough evidence to make that possibility worth investigating further.
